Occupational Therapy

A client-centered evaluation combining a targeted home safety review with a functional assessment. Ideal for clients seeking insight into both their home environment and their ability to perform daily activities.

What’s included:

  • Focused home safety check (streamlined version of full Home Safety Evaluation)

  • Assessment of functional transfers (bed, toilet, chair, shower, car, etc.)

  • Screening of Activities of Daily Living (ADLs). Areas may include bathing, dressing, hygiene and grooming, toileting, home management, leisure, light meal prep, medication management)

  • Recommendations for adaptive equipment and mobility aids

  • Customized strategies to improve strength, coordination, confidence, and independence

  • Caregiver training and education to support safe, effective care

Note: A physician referral is required for occupational therapy services provided for medical purposes, such as rehabilitation after injury, surgery, or illness.

Pre-Evaluation Prior to Elective Surgery

A focused, proactive in-home assessment to prepare your home and daily routines before elective surgery, such as joint replacements. We evaluate functional abilities, assess home safety, and provide training in compensatory techniques and adaptive strategies. Recommendations are tailored to your doctor’s post-operative guidelines to support a smooth recovery and reduce injury risk.

Single OT Session 45-min one-on-one session for functional training, equipment use, ADL retraining, caregiver education, or safety follow-up. $275 / visit
10-Visit OT Package Ten 45-min follow-up sessions. $2,500 total ($250 / visit – Save $250)
